Happy New Year! We hope you all enjoyed the holiday season & found some time to rest. The Leadership Team is excited to announce that the Portland Health Equity Alignment’s complete Social Determinants of Health Accelerator Plan is now available!
Below is a list of resources to help you navigate the plan:
1. Full Social Determinants of Health Accelerator Plan
Contents include background information, an overview of the planning process & approach, high-level recommendations for the Alignment, individual recommendations for each committee, as well as suggestions for implementation, evaluation, & sustainability of the plan.
2. 2-page Overview of Recommendations handout
This handout breaks down the core elements of the Accelerator Plan. It includes a summary of high-level recommendations for the Alignment & key components of each committee's recommended action items.
3. 12/19/24 Accelerator Plan Presentation recording
This recording is from a special Alignment meeting held on December 19th, where both Syra Health & Portland Public Health presented an overview of the plan to members & answered preliminary questions.
Both the full Accelerator Plan & 2-page overview can be viewed & downloaded on the Civilspace homepage under “Supporting Documents.” Copies of each committee’s individual recommendations can be found in your committee’s Google Drive folder or by contacting your committee chair(s).
Thank you all for your collaboration on this project over the last several months. We would also like to once again thank our health equity consultant, Syra Health, for the time & expertise they dedicated to the development of this plan. We are so glad to finally be able to share a final (working!) product with you all, and are looking forward to an exciting & successful year for the Alignment in 2025.
