
Community Health Improvement Planning
Portland Public Health is proud to present the 2022 - 2025 Community Health Improvement Plan (CHIP). Published in 2022, the CHIP is a living document that aligns community health priorities and provides a pathway to improve community health concerns.
The CHIP offers data-driven opportunities for partners to align and spearhead work related to key community-identified priorities.
The strategies adopted in the CHIP focus on three priority areas:
- Behavioral Health
- Nutrition, Physical Activity, and Weight
- Safe and Accessible Housing

In addition to the three priority areas above, access to care and health equity as they pertain to all areas will be prioritized. Portland Public Health has often heard from partners and stakeholders the belief that every member of the community deserves the opportunity to be healthy. We agree and believe that success in community health improvement has be centered on equity. In recognition of this, all CHIP strategies will strive to improve health by equalizing opportunities for those with worse health and fewer resources.
The 2022 - 2025 CHIP includes the following strategies centered on the social determinants of health:

- Healthy Minds + Bodies
