This meeting represents a culmination of several months of collaboration between the Alignment & health equity consultant Syra Health. This work was funded by the CDC’s “Closing the Gap” grant awarded to Portland Public Health (PPH) in 2023. The main goal of this project is to develop an implementation-ready SDOH Accelerator Plan focused on improving Social Connectedness & Community-Clinical Linkages for Black or African American individuals and Hispanic or Latino individuals in Greater Portland.

We began our work with Syra Health in June 2024, when Syra joined the Alignment’s spring quarterly meeting to present the project & collect preliminary data on the top barriers Alignment committees identified in the community related to Social Connectedness & Community-Clinical Linkages. After conducting surveys, facilitating breakout sessions, hosting listening sessions with Alignment committees, & working closely with Committee Chairs & PPH, Syra Health & the Leadership Team are excited to present their findings & recommendations to the Alignment in a special meeting. We will plan to host a regular quarterly meeting of the Alignment in early 2025.
