2025 Annual Report - Health & Human Services

Welcome!

The City of Portland is proud to share the 2025 Annual Report for the Health & Human Services Department, a comprehensive reflection of the department’s commitment to enhancing the well-being of our diverse and dynamic community. Through a wide range of programs and services—spanning housing stability, public health initiatives, building age-friendly communities, and more—the department continues to address Portland’s most pressing challenges with compassion, diligence, and innovation. This report highlights key accomplishments over the past year and outlines collaborative efforts to improve outcomes for all residents.

As we assess the progress made in 2025, input from Portland residents remains invaluable in shaping the City’s ongoing strategies and programs. From placing hundreds of unhoused individuals into stable housing to administering thousands of life-saving vaccines and expanding services for older residents and individuals with disabilities, these milestones were achieved in partnership with the community. Your voice and engagement will continue to be integral as we advance our shared vision for a healthier, more equitable Portland.

"I am proud of the meaningful progress our team made in 2025. Through creative problem-solving, strong community partnerships, and unwavering dedication, we have expanded critical services, helped hundreds secure stable housing, strengthened overdose prevention, and advanced our commitment to aging with dignity. This work reminds us that real change happens when we come together—progress is possible, even as we continue to confront the important work still ahead."
